What to expect
The tool reads the text on each page, groups it into lines, and guesses headings from font size. Bold fonts stay bold when the PDF names them that way.
It will not match the PDF. Columns, exact spacing, text boxes, and complex layout come out as ordinary paragraphs. That is the same honesty as Word to PDF in the other direction: good, not perfect.
A scanned PDF has no text layer. Run OCR PDF first, then convert. The tool says so instead of handing you an empty document.
